Annual Health Checks – Every year, we invite our patients with learning disabilities to attend a comprehensive health check. During this appointment, we talk through all aspects of your physical and emotional wellbeing and update your Health Action Plan to ensure it reflects your current needs.

‘Easy Read’ Information – We provide health information in easy read format to make things clearer and more accessible. This includes guidance on regular NHS screening programmes, such as:
